Model release

Where can I get a model release??? I have some good photos to be used with stock, but I would need model releases... I looked on here but could find none... could someone please point me in the right direction???

Go to the homepage: www.crestock.com At the bottom of the page, click on Legal,and yoy will get to the model release page.
