| | When your photo includes a recognisable person, you need to attach a signed model release when you upload your photo. The model release confirms that the person agrees that you submit the photos of them to Crestock. Images of recognisable people without a model release will unfortunately have to be rejected.
Just scan in or take a photo of the signed model release and submit it with the photo (there is an option to attach a model release during the upload process).

Can I take Crestock's Model release model and change the header and all the bits in the text to my own details? That would be very helpful in case I need to use the same release somewhere else in future. I might not be able to contact the model in future, thence losing any chances of using the same image anywhere else...

It is no problem to use another model release form, or change the text in our model release to include your name, address etc. Put simply, a model release just needs to say that the model allows the images to be used commercially without the photographer having to obtain further permission for specfic uses.
Just make sure you do the changes to the document BEFORE you get it signed though, not in Photoshop afterwards. If you change the content after it has been signed, you would technically speaking be forging a contract.

Beyond understanding how to read it, it's also important to understand why and when do we need the model release form . By
law, we are only required to use it when the photo intended for
commercial use, but to be on the safe side, it is recommended to always
sign one. it's a waiver from future "troubles". there is the short
standard version for the form (as published here : Model Release Form Explained ), but sometimes you
will need something more specific to the task, and should change it to
cover the occasion .
